  1. Wellness Screening

     It is important for adults to have a preventative health check every year to discuss the following items:

    • Review of current health conditions
    • Medication review and renewal
    • Review cancer screenings (e.g. colonoscopy, mammogram, etc.)
    • Review immunizations that may be due
    • Bloodwork if applicable (e.g. cholesterol, Hgb A1C, etc.)
    • Depression screening
    • Fall risk screening (if over 65-years-old)

  2. Chronic Conditions

    We assist in managing the following chronic medical conditions:

    • Asthma, allergies, COPD
    • ADHD, anxiety, depression
    • Acne and other skin conditions
    • Diabetes (including type 2 and type 1 on injectible insulin)
    • Prediabetes
    • High Blood Pressure
    • High Cholesterol
    • Obesity (weight loss medication)
    • Thyroid issues (low and high thyroid levels)

  3. Referrals

    We can provide paper referrals to specialists (including on base).

    We can help connect you with the specialists you need to see, including urgent referrals.   

    If you are seeing any specialists and would like to review their care to further understand your work up and treatment plan, please send all relevant clinic records prior to your appointment.

Evaluation of New Conditions

We can evaluate many different symptoms and concerns, such as:
